- Hazard was recently crowned Blues player of the season after scoring 16 goals and 15 assists all season long
Chelsea star Eden Hazard has reportedly reiterated his desires to join Spanish League giants Real Madrid this summer.
The Belgian attacker has been heavily linked with a move to join Los Blancos having failed to sign a contract extension to remain at Stamford Bridge beyond next campaign.
He scored 16 goals and 15 assists after 37 Premier League appearances this campaign - and he was crowned the Blues best player of the season last week.
SunSport reports that the 28-year-old admitted: "I have made my decision. Yes, I told the club a couple of weeks ago."

The publication claims the west London side have slapped a £100 million price tag on the Belgian but his manager Maurizio Sarri believes the club might not sell the forward.
Chelsea finished one spot ahead of Tottenham on the log after their 0-0 draw against Leicester City at the King Power Stadium in their last match on Sunday.
And the boss, Sarri declared: "I know very well that Eden tried to do his best for seven seasons at Chelsea.
“He has become one of the best two or three players in the world. But I think also that we have to respect him and his decision.”
Sarri expressed his desires to see the attacker extend his stay at the club, but insisted that he will also respect the player's decision should he decide to leave this summer.
"I hope, of course, like every fan of Chelsea, he will remain with us. But at the moment I don’t know."
Hazard could play his last game for the Blues in their Europa League final against Arsenal at the Baku Olympic Stadium on May 29.
Hazard blew a kiss and gave a thumbs-up sign to Chelsea fans who chanted: "Eden Hazard, we want to you stay" after the final whistle at the King Power.

Asked if his future would be cleared up after the final in Baku, Hazard replied: "I think so. I have made my decision but it is not just about me. We have a final to play and then I will see."
Hazard also revealed that he would have preferred to get the issue cleared up earlier in the season.
"I wanted that, but that’s not happened. When you are on the pitch, you try to be focused. That’s it. I just try to win games."
Legit.ng earlier reported that Eden Hazard is close to agreeing personal terms with Real Madrid over a possible move to the Spanish capital this summer.
The 28-year-old, who joined Chelsea from Lille in a deal around £32 million in 2012, has just one more year on his current deal with the club.
He has however been offered an improved contract to extend his stay at the Stamford Bridge, but the attacker turned them down a deal to become the highest-paid player in Premier League history.
